If you’re already a Chase bank member, you can log into your account and request the card without filling the online form. If you’re not a bank member, click on “apply as a guest” and follow the step-by-step guide provided by the institution.

You’ll need to provide personal and financial information, as well as contact details, your SSN, employment situation and more. Make sure to read and agree to their Terms & Conditions before submitting the formal application. 

Once you send in your form, the bank will evaluate your financial profile and should contact you with further details about whether you’ve been approved or denied.
